Agave Family Medicine contracts with most major insurance plans, including University Healthcare and Banner University Care AHCCCS plans and Kid's Care.

We currently are NOT contracted with the following plans:

AHCCCS Arizona Complete

We have self-pay plans for those who are under or uninsured.

We are committed to ensuring that every patient is able to have a physician that cares, regardless of income, insurance status, race, ethnicity, gender or gender expression.
